ARIZONA VISTA FAMILY MEDICINE TAKES MOST INSURANCE.
Please read the following and check the list at the bottom of the page. If you are a new patient, ALWAYS call and check to see that your plan is a currently accepted plan before making an appointment.
This is a Private Medical Practice and all patients are seen by appointment only. We are not an Emergency room or an Urgent Care facility; if we feel you require these services we will call 911 if medically necessary.
All new and established patients must have a valid, current insurance card with their correct name and current address. Patients without an insurance card will not be seen. Arizona Vista Family Medicine reserves the right to directly bill patients who present terminated insurance cards, or any card with incorrect information. It is the Patients responsibility to ensure that their insurance information is accurate and current.
We do all insurance and patient billing in-house and handle ALL questions.. This includes any adjustments, questions and disputes. Please note that Arizona Vista family Medicines providers only order medically necessary tests; YOUR insurance, however, has the right as the insurer to not cover that test. Speak with your provider regarding your risks and consequences should you chose, for reasons of insurance coverage, to not undergo a medically recommended test.
Medicare insurance users should note that many imaging facilities and laboratories will have you complete a "Notice to beneficiary" form which names the test that your provider has ordered and asks you to sign an agreement that, if Medicare should decide NOT to cover that test or procedure, that You agree to pay for the services. That agreement is between you and the test or imaging center. Again, your medical provider does not order "unnecessary medical tests", however, any insurance, including Medicare, may deem any test/procedure as a "not covered" service; Medicare uses the phrase "medical necessity" on their "Notice to Beneficiary".
